Why have Portal Account?

Because it’s the quickest and most convenient way to obtain test results, check the status of a prescription, and more. When logged into the portal, patients can:

  • See if a prescription has been sent to the pharmacy. They can also view the details of the prescription and other past medications.

  • View test results and lab reports.

  • View billing statements and pay balances.

  • Update contact and insurance information.

In order to create a portal account, patients must be registered in our Electronic Health Record (EMR) and receive a link from our office which can be requested via the form below or by calling 773-832-8700. Since this creating a registration link is a manual process, forms submitted outside of normal business hours will be responded to the next business day.
