Why have Portal Account?
Because it’s the quickest and most convenient way to obtain test results, check the status of a prescription, and more. When logged into the portal, patients can:
Request a prescription or see if a prescription has been sent to the pharmacy. They can also view the details of the prescription and other past medications.
Communicate with your provider. You can bypass the front desk and message the clinical team with a portal message. Portal messages are responded to within 2 business days.
View billing statements and pay balances.
Update contact and insurance information.
In order to create a portal account, patients must be registered in our Electronic Health Record (EMR) and receive a link from our office which can be requested via the form below or by calling 773-832-8700. Since creating a registration link is a manual process, forms submitted outside of normal business hours will be responded to the next business day.
