Our office is located at 2831 N Milwaukee Ave, First Floor, Chicago, IL 60618 which is near the 6 corner intersection of Milwaukee/Diversey/Kimball in Logan Square. While we don't have a designated parking lot, there is plentiful metered street parking directly in front of the building.

We are currently accepting new and existing patient appointments. Our schedule has been changing almost weekly as we respond to the COVID-19 Pandemic, so please use the Book Online button below to check availability.

Our administrative team is available on Monday, Tuesday, Thursday, and Friday, at 773-832-8700 or FrontDesk@GilleonHealth.com. We are closed on Wednsdays and weekends.

Please note, instead of calling or emailing the front desk to inquire about clinical concerns, refill requests, or the status of test results we strongly encourage patients to message the clinical team directly from the patient portal.
